Filling in an
application form for Disability Living Allowance or Attendance
Allowance can be a daunting experience. Often the questions appear
repetitive, and it can be difficult to understand exactly what
information is being asked for. It can take several hours to fill in a
single form.
DISH can
help
Our
advisors can sit down and go through the form with you, making sure
you put down the information most relevant to your claim. We will also
advise you on other information to include with your application. Just
call or email to
make an appointment to come in and see us. If you have already filled
in a form
and would like us to check it over before you send it off, you can
bring it to
our office in Papworth.
If you are unable to travel, we still may be able to help. If you
are
applying on behalf of your disabled child, our Family Advisor can visit
you in
your home.
We
also offer home visits to adults applying for Disability Living
Allowance or Attendance Allowance. Applicants for
Attendance Allowance can also be referred to a DWP advisor who will
visit you in your home. Contact
us for more details.
If
you are computer literate, you can download the form directly from
the DWP website, fill it in and send it to us by e-mail for checking.
We advise
you to contact us first by e-mail or telephone before doing so.
