If you are
computer
literate, it can be more convenient to fill in an application form
electronically, using the on-screen forms available from the Government
website. Everything you write will be neatly typed and you can easily
save a
copy for further reference.
Before
printing and sending
the form off, however, you might want to have an experienced eye to
check for
any errors or oversights.

forms on behalf of clients and will happily use our experience on your
behalf.
Just send us a copy of the file by e-mail and we will check it over and
reply
with our comments. We will not make any unauthorized
alterations to your form. Obviously, the more information we have
about
your situation the better, so you should contact us first to discuss
your
situation.
In
all cases, we recommend
you call the Benefit Enquiry Line first to obtain a standard form. This
will
have the date of the call printed on the first page. Tear off this page
and add
it to the printed electronic form to ensure any award is then backdated
to the
date of the phone call.

Note:
When downloading the forms, be careful to ensure you choose the right
one. The
ordinary, printable forms have to be printed off and filled in by hand.
Look
for the entry saying “Download the on-screen Disability Living
Allowance claim
form”. You will need to have an up to date version of Adobe Reader to
take
advantage of the form-filling features. Check by filling in the first
page,
saving and printing it. Don’t wait until you’ve filled in the whole
form to
discover you can’t print it!
